However, traditional connect cards with extra form fields can deter visitors from providing their information.
When it comes to visitor card design, the #1 rule is this:
The more form fields you display on a connect card, the fewer cards you'll have filled out.
Here’s our modern take on the church connect card:

This design is shaped like a postcard instead of a flyer to project friendliness over promotion.
And by color coding the card, it’s easy for visitors to identify it when we say:
"Pick up the blue card and scan the QR code on the back."
When the QR code gets scanned, we take guests to a hosted page on our website where we ask for their information one step at a time.
Why? Because forms that ask one question at a time convert 36% better [Source: Jotform].

Not a fan of the digital option? Swap out the QR for fill-in-the-blank form fields.
Just remember: the more form fields you display, the fewer cards you'll have filled out.
So try to keep it to 3 form fields or fewer.
One more tip for you:
To truly emphasize your connect card in service, I recommend having a matching slide on the screen while you talk about it.
It makes the card easier to identify. And guests can even scan the QR code on screen directly from their row if they don’t see a card close to them.
